INFORMATION ON THE PROCESSING OF PERSONAL DATA BY THE PROVIDER
Personal data will be processed by the provider Lucia Nemcová, DiS, with registered office in Prague 6, Verdunská 725/23, ZIP code: 16000, ID number: 11827343 (hereinafter referred to as the provider) in accordance with Regulation (EC) 2016/679 of the European Parliament and of the Council of 27 April 2016, on the protection of natural persons in connection with the processing of personal data and on the free movement of such data and on the repeal of Directive 95/46/EC (hereinafter referred to as the "Regulation" ), and other relevant regulations for the protection of personal data.

Personal data will be processed by the provider itself, with its own resources, or using the services of the following processors: provider of the XDENT information system and related applications, Infinity Energy, s.r.o., with registered office at Slavíkova 284/2, Bukov, 400 01 Ústí nad Labem, ID number: 273 37 871. Personal data may further be made available to persons who have a legal reason for accessing personal data (e.g. law enforcement authorities or other supervisory authorities with legal authority to access information), or to other persons if this is necessary for protection of the Provider's rights (e.g. court).
The purpose and method of personal data processing The Patient's personal data will be processed by the Provider for the following purposes: a. For the purpose of providing health services to the Patient and fulfilling the related legal obligations of the Provider (in the sense of Article 6, paragraph 1, letter c) and Article 9, paragraph 2 letter h) Regulation), in particular the management of medical documentation about the Patient according to Act No. 372/2011 Coll., on health services and the conditions of their provision (the Act on Health Services), as amended.
The processing of your personal data is necessary to respond to your request stated in the form and to carry out other measures before concluding a contract between you and the operator, and it is therefore a mandatory provision of data.
According to the Regulation, the patient has the right, in particular, to obtain access to this personal data, modification and deletion, the right to transfer this information in a common machine-readable format to another administrator, without the operator preventing it, the right to file a complaint with the supervisory authority if you believe that the processing of your personal data violates the Regulation. By sending a question or an order from the online form, you confirm that you are familiar with the terms of personal data protection and that you accept them in their entirety.
The administrator is authorized to change these conditions. It will publish the new version of the terms of personal data protection on its website and at the same time send you the new version of these terms to your e-mail address that you have provided to the administrator.

By paying the reservation fee, your date is valid.
CANCELLATION OF FEES: 2 days before the deadline, a reminder will be sent to your email address and mobile phone number. In case you cannot attend on the date of your choice, please contact the provider Lucia Nemcová, DiS.
If an alternative date is not arranged no later than 48 hours before the originally agreed date or if you do not show up for the ordered date for some reason, the reservation fee will be forfeited to the benefit of provider.
By creating and sending a reservation, you agree to the above-mentioned conditions for the provision of dental hygiene services by the provider Lucia Nemcová, DiS.
